New Patient Information

 

Welcome to the Center for Bone and Joint Disease. We are committed to world class quality care and expedient service for all our patients.  The following information will facilitate your first visit with us.

 

Pre-authorization.  Some insurance plans require pre-authorization and/or referral from your primary care physician before we can see you.  Our office staff will be happy to assist you with pre-authorization when you make your appointment or as soon as possible before your appointment date

New patient registration forms are available for download.  If you are planning your first visit with us, please print and complete the forms prior to your arrival.  Please list your medications with accurate milligram dosages as well as your allergies.  New Patient Forms (Acrobat)

Minor patients.  A parent, guardian, or adult with  power of attorney for consent to treat must remain in the building with minors under the age of 18 years during the entire visit.  Adult siblings, relatives and friends will require a power of attorney.  Please do not drop the child off or leave the child in the office unaccompanied. 

Imaging films and reports.  Be sure to bring all X-ray, CT scan, Bone scan, and MRI films and reports.  You may be asked to reschedule your appointment if these are not available upon your arrival.

Reports of services from other providers are frequently helpful.  These may include recent lab results, office notes, operative reports, hospital consultation notes, and hospital discharge summaries.  A form to request release of medical information is available for download. What else to bring.  Please bring ambulatory aids to show us such as cane, crutches, walkers and braces, splints or other orthotic devices you are using.

What to wear.  It is generally advisable to wear loose fitting clothing, especially if there is a chance you will need a cast.  We recommend tank tops for upper extremity and shorts for lower extremity problems weather permitting.
